Erectile dysfunction treatment: causes and options
Erectile dysfunction treatment starts with the cause. The NIH lists blood-vessel disease, diabetes, nerve and hormone problems, medicines, stress and lifestyle. Oral PDE5 inhibitors such as sildenafil and tadalafil improve blood flow to the penis. Mendoxa's prescribed 3-in-1 tablet starts at $129 a month.

Erectile dysfunction symptoms
The NIH (NIDDK) lists three: getting an erection sometimes but not every time you want sex; getting an erection that does not last long enough for sex; and being unable to get an erection at any time. It adds that ED may be a symptom of another health problem.
Erectile dysfunction causes
| Group | Examples the NIH lists |
|---|---|
| Heart and blood vessels | Atherosclerosis, high blood pressure, stroke |
| Chronic conditions | Diabetes, chronic kidney disease, obesity, COPD |
| Hormones | Low testosterone, thyroid imbalance |
| Nerves | Multiple sclerosis, spinal cord injury, pelvic surgery |
| Medicines | Antidepressants, blood pressure medicines and diuretics, antihistamines, some pain relievers, sedatives |
| Mental health | Anxiety, depression, stress |
ED can also be an early warning for the heart. In a JAMA study of 9,457 men aged 55 or older, men with ED had a hazard ratio of 1.45 for later cardiovascular events (95% CI 1.25–1.69). The authors concluded ED “should prompt investigation and intervention for cardiovascular risk factors.”

PDE5 inhibitors for erectile dysfunction
The NIH describes PDE5 inhibitors as oral medicines that improve blood flow to the penis and may help you get and keep an erection. The FDA-labeled ones are sildenafil, tadalafil, vardenafil and avanafil.
| Medicine | Label start | Label timing | Half-life |
|---|---|---|---|
| Sildenafil (Viagra) | 50 mg | About 1 h before; 30 min to 4 h | About 4 h |
| Tadalafil (Cialis) | 10 mg, or 2.5 mg daily | Effect vs placebo up to 36 h | 17.5 h |
| Vardenafil | 10 mg | About 60 min before | About 4–5 h |
| Avanafil (Stendra) | 100 mg | As early as about 15 min before | About 5 h |
The Viagra and Cialis labels list the same hard stops. Do not use them with organic nitrates in any form, or with a guanylate cyclase stimulator such as riociguat. Do not use them if sex is inadvisable because of your heart. Get emergency care for an erection that lasts more than 4 hours. Stop and get care for a sudden loss of vision in one or both eyes, or a sudden decrease or loss of hearing.
Other ED treatment options
The NIH also lists lifestyle changes (quitting smoking, limiting alcohol, more activity, a healthy weight), counseling, testosterone when it is low, injectable medicines and urethral suppositories, vacuum erection devices, and surgery when other options fail.
Where Mendoxa fits
Mendoxa is one path: an online visit that may end in a compounded rapid-dissolve tablet with sildenafil, tadalafil and apomorphine. The FDA says compounded drugs are not FDA-approved, and that the FDA does not verify their safety, effectiveness or quality before they are marketed. That applies to the Mendoxa tablet and to every other compounded ED product on this page. A clinician can say no, and should, if your heart history points elsewhere first.
